What Our ADHD Evaluation
Includes
- Clinical Interview & Behavioral Observation – A Master’s-level clinician meets with you to gather history, current symptoms, and context for how ADHD may be affecting daily life.
- Creyos ADHD Protocol – A 30 to 45 minute online assessment measuring attention, working memory, and impulse control through cognitive tasks rather than self-report alone. It must be completed on a computer or tablet, not a phone.
- Conners-4 or CAARS-2 Rating Scale – Conners-4 is completed by teachers or observers for children and adolescents ages 6 to 18. CAARS-2 is the adult version, completed as a self-report or observer form for ages 18 and up.
- Results & Diagnosis – You meet with a qualified provider who reviews every piece of data, confirms a diagnosis if appropriate, and discusses next steps, including medication management if that is part of your plan.
The Creyos Assessment: Why It
Matters
Creyos (formerly Cambridge Brain Sciences) replaces guesswork with objective performance data. Instead of relying only on questionnaires, it uses a series of short, engaging cognitive tasks to measure attention, memory, and executive function, the areas most commonly affected by ADHD. The result is a clinically validated, research-backed report that helps our providers make an informed diagnosis rather than a subjective judgment call.
Who This Assessment Is For
Children and adolescents. Combined with teacher-completed Conners-4 ratings, this evaluation captures how ADHD shows up across home, school, and social settings.
Adults. Many adults were never evaluated as children and spend years attributing ADHD symptoms to stress, personality, or simply “not trying hard enough.” The CAARS-2 and Creyos combination gives adults a clear, documented answer.
Cost and Insurance for ADHD
Testing
Kentucky Medicaid covers 100% of the ADHD assessment cost. Kentucky Services requests prior authorization from insurance before testing begins; if authorization is denied, private pay is required to complete the assessment.
Commercial Insurance
- Simple Protocol ($250 total): Clinical interview, Creyos, and Conners-4/CAARS-2
- Comprehensive Protocol ($500 total): Adds a KBIT-2 IQ test and Vineland-3 adaptive assessment
- Note: This fee is separate from any office visit copay required by your plan.
Private Pay
- Simple Protocol ($500 total): $100 clinical interview, $250 Creyos and rating scales, $150 provider follow-up
- Comprehensive Protocol ($750 total): Adds a KBIT-2 IQ test and Vineland-3 assessment
What Happens After an ADHD
Diagnosis
Once your provider confirms a diagnosis, you will discuss the right next step for your situation. That may include starting medication management services, beginning individual counseling to build coping strategies, or, for children, documentation to support school accommodations. Many families and adults move directly from assessment into ongoing care with the same clinical team.
